High risk cataract surgery in a pediatric patient. Posterior polar cataract with deficient posterior capsule. Mittendorf opacity with anterior hyaloid opacities. Posterior capsulorhexis performed with dry aspiration of opacities, avoid rupturing anterior hyaloid, and posterior optic button hole fixation of the intraocular lens.
